Electronic health record (EHR)
Definition
An electronic health record (EHR) is the digital record of a patient's health information kept by a practice or hospital, including diagnoses, medications, results and visit notes. EHRs also hold the referral and scheduling tools that depend on accurate provider data.

How do EHRs relate to provider data?
Clinicians make referrals from inside the EHR. If the provider list it searches is out of date, referrals go to providers who have moved or don't take the patient's plan.
Can EHR data verify provider information?
Some of it helps, such as where a clinician is scheduled to see patients. Most EHR data is patient health information, which is protected under HIPAA.
Interoperability
Moving data between EHRs, payers and apps depends on shared standards such as HL7 FHIR.
