Care navigation

Definition

Care navigation is helping a patient or plan member find and get to the right care: the right provider, in network, nearby, available and suited to their needs. It depends on accurate provider data.

Who does care navigation?

Health plans, benefits companies, employers' navigation services, health systems and digital health apps, through people, search tools or both.

Why does provider data matter?

Every recommendation is only as good as the provider record behind it. Sending a member to a doctor who has moved, doesn't take their plan or doesn't treat their condition wastes their time and erodes trust.

What does good navigation data include?

Verified locations and phone numbers, current plan acceptance, availability, and what each provider actually treats, not just their listed specialty.
